We have the pleasure to inform you that,\u00a0the ICAI-Bhubaneswar Chapter\u00a0has\u00a0renewed the\u00a0\u00a0MOU\u00a0with\u00a0Apollo\u00a0Hospital, Bhubaneswar for Health Checkup and Hospitalization at concessional rate for a further period from 1^{st<\/sup>\u00a0October, 2022 to 30th<\/sup>\u00a0September, 2023.<\/strong>\u00a0The copy of the\u00a0MOU\u00a0is enclosed for your kind information.<\/p>\n}

 <\/p>\n

In order to avail the concessional rate benefits provided by\u00a0the\u00a0Apollo\u00a0Hospital, a declaration of dependent family members is prerequisite to issue an authorisation letter by the Chapter. If you are interested , please fill up the form (enclosed) along with one passport photograph of yourself to issue an authorization letter which is required to produce during health check up\/hospitalization in reference to \u00a0MOU for availing concessional rate.<\/p>\n

It may be noted that, the amount of expenditure for health check\/hospitalization in\u00a0Apollo\u00a0Hospital shall be\u00a0borne by Self. This is the only initiation taken by the Chapter to facilitate the members and their dependent family members to avail the health check up\/hospitalization at a concessional rate.\u00a0\u00a0<\/strong><\/p>\n
